Definitions of the Titles on the Form

Owner is a function within the LCFTRA. Select yours from Dropdown Menu.
Note is why we are giving this FJA. Select from Dropdown Menu.
Last Name ofthe person the FJA will be delivered to.
Address First Name is the full name of the person the FJA will be delivered to.
Address Company is the Company Name. Leave Blank for Individual.
Street, City, State, Postal Code are self explanatory.
FJA Salutation is how you want the Name shown in the body of the FJA.
Delivery Notes is either Mail or who will hand deliver the FJA. Enter PDF if the FJA is to be e-mailed.
Email Address must be entered if the delivery mode is PDF.
Date Updated. Click on Symbol to right and Click on date.
